If you don't have a printer, write your order on a piece of paper. Make a list that includes the item number, number ordered, product description, price each, and total for each product ordered. Subtotal your order, add applicable sales tax, and shipping; then TOTAL.
Write your name, customer # (if you have one from a previous order), address (city, state, zip code), whether you live inside or outside the city limits, phone number, and email address. Orders cannot be shipped to a P O Box. Give complete shipping address if different from mailing address. Include a check or money order payable to Watkins Incorporated. If you prefer to use a credit card, VISA, Mastercard, Discover, and American Express are accepted. Include your credit card number with expiration date (month and year) and your signature. Shipping is free for orders of $150 US or Canadian.
